For your convenience, we accept cash, check, Visa, MasterCard, Discover, Care Credit and select insurance plans. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at time services are rendered. Payment is considered late if not paid in full within 30 days of the provision of services. Patient agrees to pay additional charge of 1% (or the maximum percentage allowed by law, whichever is lower) of the invoiced amount per month for any payment received by Michael Timothy Gocke D.D.S., P.C., d/b/a Virginia Oral, Facial & Implant Surgery more than 30 days from the date of invoice or bill for services. In the event collection of patient's account is referred to an attorney, patient agrees to pay all expenses incurred by Michael Timothy Gocke D.D.S., P.C., d/b/a/ Virginia Oral, Facial & Implant Surgery in effecting payment specifically including attorney's fees of 33% of the principal due and owing. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will send you a monthly statement.
Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but that must be implemented prior to the actual procedure.
If you need to cancel or reschedule your office appointment, please call us 24 hours in advance. If you fail to do so, or are not present at your scheduled appointment, there will be a $25.00 charge.
